Legazpi City requires health declaration form for travelers

LEGAZPI CITY, Oct 27 (PIA) - All inter-regional inbound and outbound passengers for all modes of transport in Legazpi City are required to fill-up the Health Declaration Form (HDF) as part of the updated precautionary measures of this city to ensure health and safety of the riding public.

The HDF is intended to collect basic information about the passenger, travel history, transport details and health condition. This form shall be made available in all bus terminals, airport and at the Albay Transition Facility.

"All inter-regional inbound and outbound passengers shall accomplish this form individually before embarkation, and at the Albay Transition Facility for those inbound/outbound passengers using private or rented vehicles," Rosal said

Rosal said that the HDF is mandatory for all travelers including locally stranded individuals (LSIs), returning OFWs (ROFs), and authorized persons outside residence (APOR).

The form shall be submitted to the designated staff at the terminal of final destination, and at the Legazpi City Community Home Quarantine for inbound passengers using private or rented vehicles.

A database shall be developed out of the information collected for future uses, such as contact tracing.

Furthermore, Rosal clarified that the HDF is not required for inter-province travels within the region.

"Since the Bicol region is under Modified General Community Quarantine (MGCQ) status, the IATF guidelines does not require this form for travelers within the region since the provinces have the same quarantine status, " Rosal said.

Required travel documents

As required by the provincial government of Albay, some travel document requirements remain in place.

For LSIs and APORs on indispensable land travel, required are travel authority issued by the Philippine National Police (PNP) and medical clearance certificate from the health officer of the place of origin.

For ROFs on land travels, needed are DOH-Bureau of Quarantine Certification, medical clearance certificate, negative RT-PCR result and valid ID or passport.

APORs on official travel must present approved travel order issued by the company, medical clearance certificate from the health officer of the place of origin and valid ID.

All travels by air require the above stated requirements along with a negative rapid or swab test result that is not more than three days old at the time of the flight. (SAA-PIA5/Albay)
